Knit a swatch at least 6"×6" using your project yarn and needles in the stated stitch pattern.
Wash and block the swatch before measuring — many yarns change size significantly after washing.
Place the swatch flat. Measure from the center, avoiding the edges (which are less accurate).
Count stitches and rows within a 4" area for the most accurate result.

Adjusting for a Pattern

Patterns state gauge as "X stitches over 4 inches" — enter that in the Pattern Gauge field.
